We provide a variety of home health care services to meet your individual needs. From skilled cares to personal assistance, our services are available 24 hours a day, 7 days a week.

We accomplish this through our hiring process. All applicants are carefully screened for training and experience that meets the needs of our patients, references are checked, and licenses and certificates are verified and background studies are performed. We have a diverse pool of home care personnel to meet our clients' ethnic and cultural preferences. of our clients. We provide home health care services mainly in the seven county metro areas, but also in the greater Minnesota locations.

Employment

We employ Registered Nurses (RN), Licensed Practical Nurses (LPN), Home Health Aides (HHA), Personal Care Assistants (PCA), Homemakers, Live-in-Aides, Physical Therapists (PT), Occupational Therapists (OT), Speech Therapists (ST), and Medical Social Worker.

Assessment

Initial evaluations and assessments to begin services and case management are done by Registered Nurses. “Therapy only” cases are routinely managed by a therapist, but first assessed by a Registered Nurse. Divine HealthCare utilizes a case management system to provide comprehensive coordinated care and services to clients served in the Twin Cities

Divine HealthCare is a Medicare Certified, Class A Licensed home care provider. In addition, Divine HealthCare is enrolled as a personal care and waivered services provider with Minnesota Health Care Programs. We also have County contracts with Hennepin, Ramsey, Anoka and Washington counties and other counties on a request basis. Divine HealthCare is contracted to provide home care services for various insurance companies such as Medica, Health Partners,
U-Care, Blue Cross/Blue Shield, United Health Care and Tri-West.

We accept Medicare and Medicaid clients, HMO’s or private insurance and private paid individuals.
