(RN’s) and Licensed Practical Nurses (LPN’s) provides a wide
range of personal care services within a person’s home. Our services
include the Minneapolis – Saint Paul and Greater Minnesota Area.
These services can be:
• Short Term: Acute care
services, when a loved one is home recovering from an illness or from
an operation,
• Long term: Chronic disease
care and management for treating illnesses such as diabetes and congestive
heart failure.

• Skilled Home Nursing:
Medical Services provided by Registered Nurses (RN’s) or Licensed
Practical Nurses (LPN’s). These services may include the assessment
of physical and psychosocial needs, catheter insertion and care, diabetes
management, wound and ulcer care drawing blood samples, infusion therapy
and monitoring and training of oxygen, medication and glucose levels.
Under the nurses supervision families/caregivers are instructed in how
to follow the physician’s orders to obtain the maximum healthcare
for the patient.
• Supportive Home Services:
These are medical services that extend the ability of patients to live
in their own homes, because such patients have health problems or have
difficulties to perform their day to day activities. These services
are provided by paraprofessionals such as Home Health Aide, Personal
Care Attendants, Live in Aides, Home Makers and Companions. Services
can include assistance in bathing, dressing, grooming, feeding transportation,
companionship, running errands, chore services, light housekeeping.
• Home Infusion Therapy Services:
Combine pharmacy and nursing services to administer medications through
direct infusions or through injections.
• Medical Social Services:
Divine HealthCare Social workers improve or maintain the social
emotional and functional health status of the patient as well as enhancing
the coping skills of family members/caregivers.
• Therapy Services: Therapy
Services can be provided at home to speed recovery from illness injury
or surgery, thus aiding the rehabilitation process at home.
• Physical Therapy: Provide
exercise and instruction to the patient and family/caregivers as ordered
by the physician thus helping the patient regain physical motion and
strength.
• Occupational Therapy:
Assists the patient with self-care in the home, social and work environment,
thus helping the patient to regain day to day skills such as dressing
and feeding oneself.
• Speech Therapy: Assist
the patient in the correction of speech and language problems caused
by an illness or stroke. They also develop programs to improve or restore
communication and cognitive skills, thus helping to improve breathing
or lung functioning.
• Respiratory Therapist:
Assists the patient to improve respiratory functioning.
• Home Medical Equipment:
We assist in ordering supplies and equipment.
• Phlebotomy Service:
Licensed Phlebotomists carry out physician ordered blood draws and laboratory
work at patient’s home.
